LIFE INSURANCE

Supplemental coverage available in amount equal to basic coverage through payroll deduction. Eligible dependents have $2,000 term life coverage, $1,000 coverage on infant live birth to six months. Basic and dependent coverage provided at no cost to the employee.

403(B) Plan 
WilMed matches $.50 for each $1.00 contributed up to 4% of annual salary. Vesting occurs 20% each year of service up to 100% at 5 years. Representatives are available for consultative services.

PENSION PLAN  
Fully funded by WilMed. Must complete one year of service with 1,000 hours worked and be 21 years of age. Fully vested after 5 years.

LONG TERM DISABILITY INSURANCE  
Insures up to 60% of your monthly basic wages beginning on the 91st day of a disability upon approval from the LTD Insurance Carrier.

VOLUNTARY LIFE INSURANCE  
Voluntary permanent universal life insurance that providesbasic death benefits and a cash value accumulation account. Coverage for employee, spouse and dependent children. Available through payroll deduction.

VOLUNTARY CANCER PLAN  
Voluntary cancer plan that provides cancer benefits with a wellness exam benefit reimbursement. Coverage is available for employee, spouse and dependent children. This pre-tax benefit is available through payroll deduction.

VOLUNTARY CRITICAL ILLNESS PLAN  
Voluntary critical illness plan pays a lump sum benefit if you are diagnosed with a specified critical illness. Wellness exam benefit reimbursement included. Coverage is available for the employee, spouse and dependent children. Plan is available through payroll deduction.

VACATION  
Full-Time and Part-Time employees are eligible
Eligible Date: After probationary period of 90 days

Benefit is based on hours paid up to 80 hours per pay period.

Non-management receives the following maximum:
Completed Years of Service
    0-2 = 10 days (3.08 hours per pay period)
    3-4 years = 12 days (3.70 hours pay per period)
    5-19 years = 15 days (4.62 hours pay per period)
    20 or more = 20 days (6.16 hours pay per period)

Management receives the following maximum:
    Less than 5 = 15 days
    5 years or more = 20 days


HOLIDAYS  
There are seven holidays observed by WilMed: New Year's Day, Good Friday, Independence Day, Labor Day, Thanksgiving Day and two Christmas holidays. Full-time employees receive 8 hours; Part-time employees receive 4 hours. If a holiday is worked, the employee would bank the corresponding benefit time in payroll. Non-exempt employees who work a holiday will receive a shift differential.

SICK LEAVE  
Full-time and part-time employees accrue from the 1st hour period of 90 days employment 0.0462 hours for each hour paid during each pay period. Maximum annual accrual is 96 hours per year, with a carry over maximum of 520 hours. Any additional time would convert to sick leave payback plan (Reimburse 100%).

BIRTHDAY BENEFIT  
Full-time and part-time employees who have been employed for five or more years will receive the following time off for their birthday:
  • Full-Time:
    5-9 years - 4 hours
    10+ years - 8 hours
  • Part-Time:
    5-9 years - 2 hours
    10+ years - 4 hours
During their birth month, employees receive a free meal ticket to Cafe Court and a coupon to Friendly Expressions Gift Shop.

EMPLOYEE ASSISTANCE PROGRAM (EAP)  
Available for employees and immediate family dealing with personal or professional problems that could potentially interfere with being able to work effectively. Toll-free number available.

EDUCATIONAL ASSISTANCE  
Employee is eligible for benefit if at six months, employee receives satisfactory performance evaluations.

Employees with 6 mos - <5 years of service - $1,000 per year
Employees with 5-9 years - $1,250 per year
Employees with 10+ years - $1,500 per year
All applications must be received by Human Resources prior to completion of class.

GROWING TREE DAYCARE  
On-site daycare and after school program which is Nationally Accredited. Cost can be payroll deducted for full-time and part-time employees. First priority given to WilMed employees.

INPATIENT DISCOUNT

Occupational Health Services will provide annual PPD screenings at no cost. Optional lab work is offered consisting of a complete metabolic panel, urinalysis, hematocrit/platelet count, uric acid level and cholesterol.

PHARMACY DISCOUNT  
Purchase prescriptions/medications for employee and immediate family at reduced prices. Payroll deduction available for full-time and part-time employees.
